1. Register for Federal Employer Identification Number (EIN)

• Complete and file form SS - 4
• Obtain from IRS

2. Register for Sales and Use Tax Returns, Maine Income Tax Witholding, and Unemployment Insurance Contributions and Employer Wage Report

• Complete and file Application for Tax Registration and Unemployment Contributions booklet
• Obtain from the State of Maine Bureau of Taxation and Department of Labor

3. Have employees complete and keep in employer's files:

• Employee's Witholding Certificate Form W - 4
Obtain from IRS
• Employment Eligibility Verification Form I - 9
Obtain from U.S. Department of Immigration and Naturalization

4. Obtain Worker's Compensation Insurance

• Obtain from business' insurance carrier
